Certificate of Achievement/Accomplishment

Midwest City Council Certificates are issued to honor a Midwest City student with a significant achievement or accomplishment in an area such as academics, fine arts, leadership, or sports. Certificates may be requested in the following ways:

  • By completing the on-line form, please click here: Certificate Request Form to complete an on-online request
  • By emailing the Mayor's office at jdonaldson@midwestcityok.org with a request outlining the accomplishment or achievement, or
  • By mailing a request to C/O Mayor's Office - 100 N. Midwest Blvd., Midwest City, OK 73110 with a request explaining the accomplishment or achievement.

Certificate guidelines:

  • Recipient must be a Midwest City resident.
  • Request should relevant facts, information and/or reasons why the certificate should be issued.
  • If you would like for the Mayor or a Council member to present the certificate, the logistics must be provided and we will try our best to accommodate you.
  • The request should be submitted two weeks prior to the date needed for the completed certificate.

Please note that the City's issuance of a certificate does not constitute an endorsement of an activity, etc.
