Requirements

The company must complete an application and must have a Certificate of Insurance.

Every person operating a circus or carnival shall maintain a public liability policy and present a certificate of insurance to the city, and including the City of Midwest City as an additional insured, of not less one million dollars ($1,000,000.00) liability for injury or death of one person and two million dollars ($2,000,000.00) for the injury or death of more than one person, and one million dollars ($1,000,000.00) for property damage.

**Requires a circus or carnival that also meets the definition of a "special event" as defined in city ordinance, to meet all city special event requirements (as promulgated in city ordinance and as required in city policy).**

Each food stand (sells food and drinks) needs to have City Health License, $25, which will require a State/County Health permit. Each food handler should have a food operator’s certificate.
