Volunteering in an open admission animal shelter where all animals in need are accepted can be an emotional experience at times. For animals that are suffering from disease, injury or pose a public safety threat, many times humane euthanasia is the best or only solution. Furthermore, euthanasia does take place in this facility for space issues. While volunteers are never asked to assist in euthanasia procedures or decisions, do you understand that you may be exposed to certain aspects of euthanasia that may be considered routine for staff?

By submitting this application electronically to the city of Midwest City, I am hereby attesting that any and all information I have provided is true and correct. I hereby grant Midwest City Animal Welfare, Midwest City Police Department, City of Midwest City and any of its designees my permission to investigate further any information I have provided here. I understand that a background check will be performed based on the information I have provided and any information found to be conflicting or false in statement will immediately disqualify me from the Midwest City Animal Welfare volunteer program and could be considered perjury. Should any portion of this application become legal litigation, I am agreeing to pay any and all expenses, costs and legal fees incurred by the City of Midwest City or its associates. Furthermore, I understand that I will be required to complete further paperwork in person if selected for the program that includes a liability and confidentiality consent.
